The entry form can be completed by the employer, provider or apprentice/intern
Businesses can enter multiple times in each category
Entrants must be enrolled on an Apprenticeship or Supported Internship or have completed a programme within the last twelve months (as of 29 May)
Entrants must either be resident in Dorset or work for a company based in Dorset

You can ‘save’ the form and return to it at a later date (within 30 days). Click the ‘save for later button’ at the end of the form. Please remember to submit your email address so that a link can be sent to you. We cannot give you access to your application information, so without this link your application will be lost.

Q1: Role, duties and responsibilities
Please explain the apprentice’s role within the organisation and outline their key duties and responsibilities. This question helps the judges understand how the apprentice’s role fits within the business or organisation and provides context for their achievements. Responses may include:

An overview of the apprentice’s role and main responsibilities
The types of tasks, activities or projects they are involved in
Whether they work independently, as part of a team, or alongside other apprentices
The size of the team or organisation they work within
Whether this is the apprentice’s first apprenticeship and how they secured it
The apprentice’s goals, ambitions and what they hope to achieve through their apprenticeship

For Supported Internship entries: This may include the type of placement, the activities undertaken, levels of support provided, and how the intern contributes within the workplace.

Q2: Contribution and impact
Please describe the apprentice’s contribution to their employer’s business objectives, the impact this has had on the organisation, and the benefits delivered.
This question helps the judges understand how the apprentice or intern has made a positive difference within the business. Contributions do not need to be large-scale or formally measured, particularly for smaller organisations. Responses could include:

Examples of where they have gone above and beyond their day-to-day duties
A specific task, project or responsibility where they have made an outstanding contribution
Improvements to ways of working, customer experience, efficiency, quality or team support
New ideas, practices or improvements they have suggested or helped to implement
Positive outcomes such as time saved, improved processes, feedback from customers or colleagues, or increased confidence and reliability
For Supported Internship entries: Impact may include improved workplace confidence, independence, teamwork, communication skills, or readiness for future employment. Judges are looking for real impact at an appropriate scale, not the size of the organisation.

Q3. Personal development and progression
Please describe how the apprentice or intern has benefited from their apprenticeship or supported internship and the impact it has had on their personal and professional development. This question is an opportunity to reflect on the journey so far and demonstrate growth, progress and future aspirations. Judges are looking for honest reflection, personal development and positive progression. Responses may include:

Skills, knowledge or confidence gained
Personal progression, such as taking on new responsibilities or overcoming challenges
Key achievements or milestones reached
How the programme has supported career ambitions and future plans
What motivates them to continue and complete the programme
How far they feel they have come since starting

For Supported Internship entries: This may include progress in areas such as confidence, independence, communication, workplace behaviour, or preparation for sustained employment.

Q4. Personal Statement: Why should you win this award?
This question should be completed by the apprentice or supported intern.
This is your opportunity to share your story and highlight what makes you proud of your achievements so far.
Please note that you don’t need to have won awards or feel comfortable “selling yourself”. Judges value honesty, effort, personal growth and positive impact just as much as high-profile achievements.
You may wish to include:

The personal qualities, attitudes or behaviours that set you apart
Achievements or milestones you are most proud of
How you act as a positive ambassador for apprenticeships, supported internships or vocational learning
Any additional activities, competitions or recognition you have taken part in
How being named Apprentice of the Year would support your confidence, development or future goals
How you would approach the next year if you were to receive the award
Tell your story in your own words and use examples where possible.
